About Office Hours
Our office staff is available weekdays 9 am to 5 pm, and one Saturday per month. Please call during weekday office hours to get more information for Saturdays.
The doctor will be answering their pager after hours. In case of medical emergency please go to the affiliated hospitals: The Brooklyn Hospital Center, SUNY Downstate Medical Center and The Mount Sinai Hospital Center.
About Medical Records and Immunizations
If you have been seen in another practice or clinic, and wish to transfer you care to our office, be advised to request transfer of your medical records prior to being seen for the first time.
If you have never been seen in this office but you are referred from a hospital or an emergency room, bring your discharge papers or any other documents given to you by the attending physician.
About Insurance Plans
If you have an insurance plan that we participate with, you must contact them prior to being seen, and arrange for us to be your Primary Care Physician (PCP). If you do not do so, then you will be responsible for the fee. Some insurance plans have deductibles and/or co-pays. It is required by these plans that payment be made at the time of your visit. You are responsible for any deductible and balance that your plan indicates on their explanation of benefits.
If we do not participate with your plan, payment is expected at the time of service. Your itemized receipt should be attached to your insurance form and sent by you to your carrier, who will reimburse you directly.
If you are referred for specialist's care, you should arrange with your PCP, to send or fax over the referral form prior to our appointment.
